Candidates seeking lateral entry admission to the Direct Second Year Bachelor of Pharmacy (D.S.Y. B. Pharm) program through the Centralized Admission Process (CAP) or Institute Level Quota must present all original credentials along with self-attested photocopies for document verification during seat reporting.

Essential Credentials (All Candidates)

  • D. Pharmacy Marksheets: Final Year & all preceding semester/annual marksheets.
  • D. Pharmacy Passing / Diploma Certificate: Proof of completion from MSBTE or approved equivalent authority.
  • SSC (Std. X) & HSC (Std. XII) Marksheets: Supporting secondary and higher secondary certificates.
  • College Leaving / Transfer Certificate (LC / TC): Issued by the previous D. Pharm institution.
  • Domicile & Nationality Certificate: Proof of Maharashtra domicile status and Indian citizenship.
  • CAP Allotment Letter: Seat allotment confirmation issued by the State CET Cell.
  • Identity Proof & Photos: Government-issued photo ID proof and recent passport-size color photographs.

Reserved Category Candidates (SC / ST / VJ / DT / NT / OBC / SBC)

  • Caste Certificate: Issued by the competent authority of Maharashtra State.
  • Caste / Tribe Validity Certificate: Mandatory validity certificate from social welfare department.
  • Non-Creamy Layer Certificate (NCL): Valid for the ongoing academic admission cycle (applicable for VJ/DT/NT/OBC/SBC).

Special Category Candidates (EWS / PwD / Defense)

  • EWS Eligibility Certificate: Proforma-V issued by the competent revenue officer.
  • Income Certificate: Valid annual household income certificate issued by competent authority.
  • Disability / Defense Certificate: Official authorization certificates for applicable horizontal quota claims.
Reporting Instructions: Candidates are required to carry three sets of self-attested photocopies, all original documents in hard copy, and scanned soft copies in PDF format during admission reporting.
